Some important benefits information:
- Every laid off employee should have received information from COBRA. If you have any questions at all, please call the COBRA customer service line at 757-340-4567 x118, or you can reach Cindy Hoggard, Senior COBRA Administrator at choggardflex-admincom.

o Former U.S. Employees who live in Washington D.C., Virginia or Maryland may file for unemployment benefits in Virginia because that’s where they work.

o You don't need any need any documents from MBN to apply for unemployment benefits. Once you apply for unemployment benefits, MBN will be notified and will supply any needed information.
For those applying for unemployment, if you would like to do it in person you can visit the Virgina Works site in Alexandria. They are at 5520 Cherokee Avenue, Suite 100. Please ask for Myrna Tejera. She knows all about our situation and has been very helpful. Myrna is there every weekday except Thursday. On Thursdays she is in the Arlington office - at 2100 Washington Blvd. Her work mobile phone is 571-319-1479. Please feel free to reach out to her with any questions.
o Former U.S. employees remain active on the health insurance plan until the end of April.

Beginning in May, you have other options including:
-COBRA - former employees who were enrolled in the health insurance plan will be sent information in the mail by COBRA
- online marketplace - such as Welcome to the Health Insurance Marketplace® | HealthCare.gov
